And then someone suggests a food sensitivity test. The response is usually one of two things: either relief — finally, an answer — or suspicion — another test to spend money on. Both reactions are understandable. Neither is fully right.

Food Sensitivity vs Food Allergy vs Food Intolerance: Getting the Terms Right
These three terms are used interchangeably in everyday conversation — and almost always incorrectly. They describe three distinct physiological processes, and the distinction determines both what's happening in the body and what to do about it.
Food allergy is an immune-mediated response — the immune system misidentifies a food protein as a threat and mounts an IgE antibody response. Reactions are typically rapid (within minutes to two hours), can range from mild hives to anaphylaxis, and are reproducible — the same food reliably produces the same reaction. This is a clinical emergency in severe cases and requires definitive diagnosis and strict avoidance.
Food intolerance is a non-immune-mediated response. The body cannot properly digest or process a specific food component — the most familiar being lactose (in dairy) and fructose (in certain fruits and FODMAPs). The response is dose-dependent — small amounts may be tolerated; larger amounts are not. Symptoms are primarily digestive: bloating, gas, loose stools, cramping.
Food sensitivity sits in the middle ground — and it is the most poorly defined of the three. It describes a broader, often delayed reaction to a food that is neither a true allergy nor a classic intolerance. Symptoms can include digestive symptoms, skin changes, headaches, fatigue, and brain fog, appearing hours or even days after eating the trigger food. This delay is what makes it so difficult to identify without structured investigation.
Getting these terms right matters — because the appropriate test and the appropriate treatment depend entirely on which category you're in.
Food Intolerance Test: Why Accurate Diagnosis Matters
Food intolerance and sensitivity are among the most under-investigated causes of persistent gut symptoms. Not because they're rare — they're quite common — but because the symptoms are non-specific, the timeline between eating and reacting is often delayed, and random elimination of suspected foods rarely provides a clear answer.
Many people with unidentified food intolerances are living with a working diagnosis of IBS, chronic bloating, or non-specific digestive discomfort. These labels are not wrong — the symptoms are real — but they describe the presentation, not the cause. If a food trigger is driving the symptoms, no amount of medication for IBS will resolve the underlying issue.
This is why food sensitivity testing — done correctly — is not a money-making exercise. It is a diagnostic tool. And like any diagnostic tool, its value depends on using the right test, for the right person, at the right stage of investigation.
The key phrase there is the right stage. Food sensitivity testing is not the first step. It is the step that becomes important when initial management — dietary adjustment, stress management, gut microbiome support — has not produced adequate resolution. Running a test before trying basic dietary and lifestyle changes first is not good clinical practice. But refusing to investigate when symptoms persist after reasonable first-line management is equally unhelpful.
How to Test for Food Intolerance and When to Investigate
Food intolerance symptoms are recurring reactions that occur after eating certain foods and do not involve an allergic immune response. Common signs of food intolerance include bloating, abdominal pain, diarrhea, constipation, excess gas, fatigue, brain fog, and skin flare-ups that repeatedly occur after consuming specific foods.
When to Consider Food Intolerance Testing
Food intolerance testing may be appropriate if you have:
- Persistent digestive symptoms: Bloating, abdominal pain, irregular bowel habits, diarrhea, constipation, or excessive gas that continue despite dietary and lifestyle changes.
- Symptoms that consistently occur after eating: Even if you haven't identified the exact trigger food.
- Persistent IBS or functional gut symptoms: Especially when standard treatment has not provided adequate relief.
- Diet-related skin conditions: Eczema, urticaria (hives), or unexplained rashes that seem to worsen after certain foods.
- Unexplained fatigue or brain fog: Particularly when symptoms appear to be associated with meals.
- Suspected food intolerances: Such as lactose intolerance, fructose intolerance, histamine intolerance, or non-celiac gluten sensitivity, where structured evaluation is needed.
When Food Intolerance Testing May Not Be Helpful
Testing is generally less useful when:
- Symptoms are sudden and severe, suggesting a food allergy that requires urgent medical assessment.
- Multiple food groups have already been eliminated without structured clinical guidance, making results difficult to interpret.
- Another diagnosed medical condition fully explains the symptoms, making food intolerance less likely to be the primary cause.
Who Should Consider Testing?
Food sensitivity investigation is most useful for adults with chronic, recurring gut symptoms that have not responded to initial management. It is also relevant for people with:
- IBS (confirmed or suspected): Particularly where specific dietary triggers are suspected but not yet identified
- Inflammatory bowel disease in remission: Where dietary triggers worsen symptoms during otherwise stable periods
- Chronic skin conditions: Eczema, urticaria, or rashes with a suspected dietary component
- Persistent unexplained bloating: That has not been explained by structural investigation
- Expanding food sensitivities: Where the list of foods causing discomfort is growing without clear explanation
- Children with persistent gut or skin symptoms: Should be investigated under medical supervision, with dietary changes guided by a clinical dietitian
The Gold Standard: Elimination and Reintroduction
The elimination diet for food intolerance is the most clinically validated method for how to identify food intolerance. Unlike blood tests, which have limited evidence for most food intolerances, the elimination and reintroduction protocol has been the reference standard in clinical practice for decades because it links symptoms directly to specific foods.
How It Works
The elimination and reintroduction process consists of three structured phases:
- Elimination phase: Suspected trigger foods or food groups are removed from the diet for 2–6 weeks. For digestive symptoms, the low-FODMAP elimination diet is the most evidence-based approach. Other elimination diets may target gluten, dairy, histamine-containing foods, or individual foods suggested by the clinical history.
- Monitoring phase: Symptoms are carefully tracked throughout the elimination period. Significant improvement suggests that one or more of the eliminated foods may be contributing to symptoms, while little or no improvement indicates another cause should be considered.
- Reintroduction phase: Foods are reintroduced one at a time, usually every 2–3 days, while monitoring for symptom recurrence. This systematic approach helps identify the specific food trigger with far greater accuracy than currently available blood tests.
Why It Works
The elimination and reintroduction protocol evaluates the body's real-life response to foods rather than relying on antibody levels or laboratory markers that may not correlate with symptoms. By observing symptom improvement during elimination and recurrence after reintroduction, clinicians can confidently identify problematic foods and create a targeted, sustainable dietary plan.
Why it needs clinical guidance: The protocol sounds straightforward but is frequently done incorrectly. Eliminating too many foods at once makes the reintroduction phase uninterpretable. Reintroducing too quickly, or in too large a quantity, confuses the results. Nutritional adequacy during a restrictive elimination phase needs to be maintained. And the emotional relationship with food — the anxiety of restriction, the relief of reintroduction — benefits from professional support. Undertaken without guidance, the elimination diet becomes another guessing exercise. Done properly, it is the most useful diagnostic tool available for food intolerance.
What About Commercial Food Sensitivity Tests?
There is an important and honest distinction to make here — one that many commercial testing services do not make clearly.
Most over-the-counter and direct-to-consumer food sensitivity tests measure IgG antibodies — immunoglobulin G — to specific foods. These are now widely available as food sensitivity test at home kits, ordered online and completed with a finger-prick blood sample. The presence of IgG antibodies to a food is often marketed as evidence of sensitivity or intolerance to that food.
The clinical evidence does not support this interpretation.
IgG antibodies to food are a normal immunological response to eating — they reflect exposure to a food, not intolerance to it. Most people who eat a varied diet will show elevated IgG to many foods, regardless of whether those foods cause symptoms. The American Academy of Allergy, Asthma and Immunology, the European Academy of Allergy and Clinical Immunology, and the Canadian Society of Allergy and Clinical Immunology all explicitly state that IgG food testing should not be used to diagnose food sensitivity or intolerance, and that dietary elimination guided by IgG results may lead to unnecessary nutritional restriction.
This does not mean that commercial tests have no role at all — for some people, a commercial test combined with clinical interpretation and a structured elimination-reintroduction phase can provide a useful starting framework. But the test result alone is not a diagnosis. Acting on a list of "reactive" foods from an IgG test without clinical guidance, eliminating multiple food groups simultaneously, is likely to cause more confusion — and potentially more nutritional harm — than it resolves.
The diagnostic tool that works is the elimination-reintroduction protocol. Commercial IgG tests are, at best, a conversation starter — not a conclusion.
IgE Food Allergy Test vs. Food Sensitivity
For completeness — if a true food allergy is suspected, the appropriate investigation is IgE-mediated testing: either a skin prick test (where small amounts of food extracts are placed on the skin and the reaction observed) or a specific IgE blood test (measuring immune antibody levels to specific food proteins).
These tests are clinically validated, widely available, and appropriate when the concern is a true immune-mediated allergic response — hives, throat tightening, anaphylaxis, significant immediate reactions after eating. They are not the right tool for chronic bloating, delayed fatigue, or intermittent digestive discomfort.
If you are concerned about a possible true food allergy — particularly if reactions are rapid, significant, or have involved throat or breathing symptoms — this warrants urgent clinical evaluation rather than self-directed dietary investigation.
How to Approach This Properly
The practical sequence for someone with persistent symptoms they suspect may be food-related is this.
Start with the basics: address gut microbiome health, stress load, sleep, and meal timing before attributing everything to a specific food trigger. If symptoms persist despite good basic management, food sensitivity investigation becomes appropriate. Seek clinical guidance before beginning an elimination diet — particularly if you are already restricting multiple food groups, have a history of disordered eating, or are managing another diagnosed condition.
A structured elimination-reintroduction protocol, guided by a clinician or registered dietitian with gut health experience, is the most reliable path to identifying specific triggers. The process takes time — typically six to ten weeks from start to a clear answer — but it produces information that actually changes what you eat and why, rather than a list of food reactions that may or may not be clinically relevant.
If testing is pursued, use it as a framework for structuring the elimination phase — not as a definitive diagnosis.
The goal is a clear answer that leads to targeted, specific, sustainable dietary modification — not an ever-expanding list of foods to avoid.
A Real Client Story
Anand was 41 when he came in. He had been avoiding gluten for almost two years — on the advice of a friend, and following an IgG food sensitivity test that had flagged wheat, dairy, and eggs among several other foods. His diet had become progressively more restrictive. He was eating very carefully. He still had significant bloating and irregular bowel habits.
